Forms
There are several forms that must be completed and in our possession before we can assume the responsibility of caring for your child. NO EXCEPTIONS. These forms contain vital information and ensure that your child will get the very best care possible. The forms are, but not limited to, as follows:

• Registration Form
• Medical Consent Form
• Medication Administration Form
• Parent / Guardian / Provider Contract
• Immunization Records
• Health Records
  

Updates
You are required to keep us informed of any change in addresses, telephone numbers, and other pertinent information listed on any/all of the above forms. If you have any questions regarding the completion of these forms, please feel free to ask.

Security Deposit
Before your child is admitted into child care, you must pay a one week security deposit. The deposit protects us from NSF checks and unexpected family departures from child care. Deposits are equal to your normal weekly fee. In the event your child does not start care by the agreed upon date or if you withdraw your enrollment less than 30 days prior to your enrollment, your position will be forfeited and your deposit to hold the position will become non-refundable .
